Online Forms

The Office offers secure online forms for individuals looking to:

  • make a protected disclosure of wrongdoing they believe is happening within the federal public sector;
  • make a complaint of reprisal; and/or
  • submit a request for funding for legal advice related to a disclosure of wrongdoing or complaint of reprisal.

To use these forms, you must have access to a valid e-mail address. Once you submit your completed form, it will be encrypted and sent directly to the Office.

For security reasons, after a period of more than 30 minutes of inactivity, your session will be timed out. You will lose all information and your form will not be saved.
