Hello,

Before you begin completing the online form, please review the information below. Following these steps will make the process smoother.

Mandate

Our Office investigates allegations of wrongdoing as defined by the Public Servants Disclosure Protection Act. The Act is not intended to address matters of a personal nature, such as harassment complaints or workplace grievances.

If you are not sure whether our Office can investigate the allegations you are reporting, ask yourself the following question: “If this wrongdoing were founded, could it damage the public’s trust in the public service?” If the answer is “no,” it is unlikely that it would lead to an investigation.

Form timing-out

Please be aware that the online form may time out if it is left inactive for too long. To avoid losing your work, we strongly recommend that you prepare your answers in advance, using a separate document. Once you are ready, you can copy and paste your responses into the form.

Contact information

The form will ask for your contact information. You will also be asked to provide contact information about the alleged wrongdoers and any individual representing you, if applicable. This includes their names, job titles and the names of the organizations where they currently or previously worked. Please have this information ready before you begin.

Supporting documentation

You may upload supporting documentation. Do not include Cabinet confidences or information that is protected by the solicitor-client privilege. To ensure your file is reviewed promptly, only provide documentation relevant to the allegations and specify which parts are relevant to the alleged wrongdoing.

You may upload up to five documents. There are requirements regarding supported formats and maximum file size.

Other complaints or processes

You will be asked whether you have already filed a formal complaint with another person or body acting under another Act of Parliament, or under a collective agreement. Examples include your organization’s Senior Officer for internal disclosure, or the Canadian Human Rights Commission. Please have this information ready before you begin.

Taking the time to prepare this information in advance will help you complete the online form.
