Instructions before completing a reprisal - video with transcription

Hello,

If you believe that you experienced a reprisal because you reported wrongdoing or participated in an investigation into a disclosure of wrongdoing, you can file a complaint with our Office.

Before you begin completing the online form, please review the information below. Following these steps will make the process smoother.

Form timing-out

Please be aware that the online form may time out if it is left inactive for too long. To avoid losing your work, we strongly recommend that you prepare your answers in advance, using a separate document. Once you are ready, you can copy and paste your responses into the form.

Disclosure file number

You will be asked to provide details relating to the disclosure, including the relevant dates and the names of the individuals or offices to which the disclosure was made. If you made a disclosure to our Office, you only need to include the file number or the reference number assigned to you in the format displayed on screen.

If you are a witness who participated in an investigation, you will be asked to upload your letter of notice.

Supporting documentation

You may upload supporting documentation. Do not include Cabinet confidences or information that is protected by the solicitor-client privilege. To ensure your file is reviewed promptly, only provide documentation relevant to the allegations and specify which parts are relevant to the alleged reprisal.

You may upload up to five documents. There are requirements regarding supported formats and maximum file size.

Other complaints or processes

You will be asked whether you have already filed a formal complaint with another person or body acting under another Act of Parliament, or under a collective agreement. Please have this information ready before you begin.

RCMP

Current and former members of the RCMP must indicate whether the subject matter of the reprisal complaint concerns an action under Section 20.2 of the Royal Canadian Mounted Police Act, or any matter that is the subject of an investigation or proceeding under Part IV of the Act.
